Medical Social Worker – Homecare Services

Job Summary:
The Medical Social Worker in Homecare provides professional social work services to patients and their families in the home setting. This role involves conducting psychosocial assessments, providing counseling and resource referrals, and collaborating with the interdisciplinary team to address the social and emotional factors that impact patients’ health and recovery.
Essential Functions
- Conduct comprehensive psychosocial assessments of patients in their homes to identify social, emotional, and environmental factors affecting health and recovery.
- Develop individualized care plans in collaboration with the patient, family, and interdisciplinary team.
- Provide counseling and support to patients and their families to help them cope with illness, disability, and end-of-life issues.
- Identify and connect patients and families with community resources, financial assistance, and other supportive services.
- Advocate patients’ needs, ensuring access to services and resources.
- Educate patients and families about advance directives, caregiver support, and community-based programs.
- Participate in interdisciplinary team meetings to coordinate care and discuss patient needs and progress.
- Maintain accurate, timely, and thorough documentation in accordance with agency policies and regulatory requirements.
- Provide crisis intervention and support in emergency situations.
- Promote patient-centered care and respect for cultural differences and preferences.
Qualifications
- Bachelor’s degree in Social Work (BSW) required; Master’s degree in Social Work (MSW) preferred.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the state of practice.
- At least one year of social work experience in a healthcare setting, preferably in home health or hospice.
Competencies
- Knowledge of home health regulations, social work best practices, and community resources.
- Excellent communication, assessment, and problem-solving skills.
- Ability to work independently in the home setting and manage a caseload.
- Commitment to patient-centered, culturally competent care
Work Environment & Physical Demands
• Ability to travel to patients’ homes, which may include exposure to various home environments.
• Ability to lift to 25 pounds and perform tasks requiring manual dexterity.
