Heart and Hand Ministries (HNH) is a 501(c)3 nonprofit organization founded in 1997 to provide support for individuals and families experiencing poverty. Located in the Belton/Raymore area, we serve the needs of the surrounding community as well as all of Cass County and parts of south Jackson County. 

Our mission is to come alongside people experiencing poverty and provide critical services and support to instill hope, encourage agency, and give people tools to build flourishing lives.

ROLE: PT Case Manager

Section 1: Overview and Scope of Position

Position Overview:
We are seeking a compassionate and dedicated Case Manager to join our team to provide holistic, client-centered solutions to individuals experiencing homelessness and/or long-term unemployment. This position is responsible for building trusting relationships with clients and helping them navigate complex systems to access housing, employment services, mental and physical health care, and public benefits such as SNAP, Medicaid, Social Security, disability, and housing vouchers.
The ideal candidate will utilize a trauma-informed, strength-based, and highly relational approach to meet each client where they are—recognizing their unique experiences, challenges, and goals.

 
Section 2: Qualifications


•Minimum of 2 years of experience working with unhoused individuals or other highly vulnerable populations and bachelor’s degree in Social Work, Psychology, Human Services, or related field required
OR
•Minimum of 4 years of experience working with unhoused individuals or other highly vulnerable populations and high school degree with strong, demonstrated administrative acumen.

•Demonstrated knowledge of trauma-informed care, harm reduction, motivational interviewing, and strengths-based practices.
•Robust understanding of public benefit systems including Medicaid, SSI/SSDI, SNAP, and housing subsidies.
•Knowledge of Cass County public service infrastructure preferred.
•Skilled at building rapport and maintaining professional boundaries with individuals from diverse backgrounds that are navigating crises.
•Strong administrative skills including proficiency in case management software, e.g. Case Worthy, Charity Tracker, VCM, Social Solutions Apricot, etc. Plus proficiency in Microsoft 365, Google Suite, and other web-based software.
Valid driver’s license

 
 
Section 3: Responsibilities and Duties

Assessment and Planning:
•Build and maintain authentic, trusting relationships with clients to foster long-term engagement and support.
•Conduct individualized, comprehensive assessments of clients’ needs, strengths, and barriers related to housing, employment, health, and income.
Regularly reviewing and adjusting care plans based on client progress and changing needs. 

Service Coordination:
•Collaboratively develop and implement personalized service plans aligned with each client's goals, strengths, timelines, and needs
•Provide hands-on assistance with navigating and enrolling in public benefit programs such as SNAP, Medicaid, SSI/SSDI, housing vouchers (e.g., Section 8), and other local/state/federal assistance programs.
•Support clients in securing and sustaining safe, stable housing through landlord mediation, housing search assistance, and coordination with housing authorities.

Advocacy and Support:
•Facilitate referrals and warm hand-offs to mental health, substance use, and primary care providers; accompany clients to appointments as needed.
•Advocate for clients within service systems and help them navigate bureaucratic and institutional barriers.

Documentation and Reporting:
•Maintain accurate and comprehensive case records. 
•Monitor client progress regularly, reassessing needs and updating service plans in partnership with the client.
•Maintain accurate and timely case notes, service documentation, and reporting in compliance with agency policies and funding requirements

Other Duties:
•Providing crisis intervention and support. 
•Educating clients about available resources and services. 


Section 5: Working Conditions and Physical Effort

  • Regular hours Monday-Friday, 9:30am –2:30pm at our program center building with some opportunity to work from home on occasion.
  • Much of the work is typically performed in an interior temperature-controlled office work environment. Position requires both prolonged periods of sitting at a computer as well as actively engaging in dialogue and interpersonal activities.  

Section 6: Compensation
This is currently a PT position that requires 4-5 hours per day. It has a strong chance to grow into a FT role but this is not required.

Salary Range: $22,000–$27,000 depending on experience and hours per day.

PTO: 2 weeks/year, Paid Holidays, Opportunity for advancement, annual cost-of-living increases, and performance based pay increases.
